View Crisis HotlinesPlanning and starting an end-of-life conversation can be challenging, whether you have received a terminal diagnosis or are caring for someone who has.
We have developed these conversation cards to help you:
– Identify and consider important end-of-life elements for you or the person you care for
– Find questions you might feel comfortable using in an end-of-life conversation, perhaps as an icebreaker.
The questions are grouped into colour-coded sets for your convenience, but you can mix and match questions from different sets. Two identical sets have been provided below: one for printing and another for viewing on your computer screen, tablet or smart phone.
When using these questions, remember to consider the feelings and specific circumstances of everyone involved, including the patient, carers, family members, and friends.
